This is the information we need to complete your Video.

Fill this form out AFTER you have made your Payment. If the form is sent in before your payment has been made it will not be valid and you will have to resubmit the form after your payment is received.

Once the Video is created and we send you the link for approval you have 15 days to approve your video or request any grammar revisions. If we do not hear from you within the 15 days of the first notice we send the video for preview we will consider the video approved and your portfolio will be closed.

NOTE* If you want a certain song/sound on the video you will need to email the MP3 file to us along with your photos. We do NOT purchase music for videos.

Music will not be revised in videos.

We do NOT do voice over for our videos. However you are more than welcome to record your own voice over (MP3 format only) and we can incorporate it into the video.

Any extras we do not have on the purchase page will need to be approved first. ie: embedded videos audio cutting/slicing etc... You will be sent a PayPal request for the extra cost for extras.

This Form MUST be filled out completely. By not filling the below form out you will delay the progress of your video. Revisions of your video will only be honored if you fill out the form completely and supply us with all information, photos, music and everything you want on your video. By Pressing the Submit button you acknowledge you have read this page in it's entirety and understand what is needed to complete your video. You also understand that if you do not send us the Video Form completed in it's entirety, Photos/Images and Music for your video you will not be entitled to revisions of your video (exceptions are misspelled words only).

You may e-mail a zip file of your Photos/Images, Music or additional comments for your video to: csyoutubecommercials@gmail.com Only send JPG, JPEG and GIF images. Music must be in MP3 or AVI format only.

Videos are made up using "Still Photos" We do NOT go into the open field and take "Real Life" videos.

Video Creations Policy is that we place a Video Creations Credit at the end of each video. If you would like OUR credits removed you can go to HERE and purchase a Credit Removal Package. If you want to have your Credits added at the end of your video you can also purchase a package for that option.

NO Refunds will be honored after the video has been created. This is because you have access to the video download when we send you the preview. We do all we can to make your video(s) to your satisfaction.
